This packet contains two form templates to be used for clients boarding elderly pets. The forms are to be signed by the client, with a copy to be sent to their veterinarian, and one to be kept on file at the boarding facility:
- One form is a “keep my pet alive no matter what” form.
- The other form is a “no heroics” type of form, authorizing euthanization if the pet should start to fail.
The great thing about these forms is that they open up the lines of communication with the client. The client knows that his pet is old, and that something could happen…but discussing and signing the forms creates a beneficial dialogue so the facility knows what to do in case the pet’s health begins to deteriorate. These are invaluable! They include a place to customize the form with your facility name as well
